Palliative and end of life care | COORDINARE - South Eastern NSW PHN
COORDINARE has acknowledged palliative and end of life care as a health priority in our region since 2015.
Recognising our regions ageing population, the growing demand for palliative care services, and the critical role that GPs play in supporting palliative and end of life care we are establishing the following initiatives.
Improving palliative and end of life care
Under the Greater Choice for at Home Palliative Care measure, COORDINARE will implement a suite of activities across the region to improve awareness of local palliative care options and facilitate access to palliative care services at home.

We are committed to working with community, general practice, LHDs and service providers to:
raise awareness of early advance care planning discussions
raise consumer awareness of local palliative care systems and support
build capacity and skills within the primary care workforce
promote and support system integration across the sector
develop a data driven approach to palliative and end of life care.

Voluntary assisted dying
The NSW Voluntary Assisted Dying Act 2022 allows eligible people the choice to access voluntary assisted dying in NSW from 28 November 2023, and will be embedded in end-of-life pathways across the region.
Patients must be informed about all options available to them including palliative care and other treatment options. Find further information at the links below, or scroll down the page to view the video library:
The Advance Care Planning Toolkit provides a practical guide for general practice teams to assist patients to navigate the advanced care planning process. It describes how to successfully implement advance care planning in a practice as a CQI activity, and for PIP QI and CPD purposes.

The CareSearchgp app supports GPs in delivering quality palliative care across the life course. It brings together guidance on terminal prescribing for specific symptoms and evidence-based information on key care issues

It can be confronting to think about issues like palliative, end-of-life care, and dying but it’s never too early to have these conversations with your loved ones and health providers. This will ensure they know what matters most to you and can respect your treatment choices.
