Care Finder

In order to support aged care across South Eastern NSW, COORDINARE is looking to commission an organisation/s to establish and deliver Care Finder services across the region.
The Care Finder services will provide support to people over the age of 65 years and Aboriginal people over the age of 50 years, who are eligible to receive aged care services, and who may be defined as vulnerable and experience challenges in accessing My Aged Care.
Care finder services will provide face-to-face, intensive support to assist this target group to understand and access aged care services and connect them with other relevant supports in the community across South Eastern NSW.
