COORDINARE is the Primary Health Network (PHN) for South Eastern NSW. We work with government, non-profit, healthcare providers, service partners and communities to coordinate local health services and support those at risk of poor health outcomes.
We envision one coordinated regional health system that provides exceptional care, promotes healthy choices and supports resilient communities.
Our leadership
Our governance embeds strong links between the Board, our GP-led Clinical Councils and Community Advisory Committee. This structure provides strong accountability and oversight into Board discussions, and to identify clear strategies which translate national priorities locally.
The organisation is led by a skills-based Board which sets the strategic direction and implementation of our strategic priorities. Members bring expertise and governance in aged care, education, research and evaluation, and private health insurance.
Our Board provides a unique blend of both private and public perspectives, and innovative thinking to govern and guide our organisation.
The Illawarra Shoalhaven Clinical Council performs a critical communication role between the Board, the Executive Team and general practitioners. They provide input and feedback into planning service responses at practice and population health level, as well as other issues.

The Aboriginal Health Council supports COORDINARE to gain insight into locally relevant Aboriginal communities’ perspectives regarding their specific health needs. This advice and knowledge aims to ensure the delivery of culturally safe services, and underpins our commitment to improving Aboriginal health and wellbeing.
Our Community Advisory Committee contribute to our understanding of local health needs and priorities in South Eastern NSW and advise on strategies for engaging with regional communities.

Who we are

Our region
COORDINARE is one of 31 Primary Health Networks (PHNs) located across Australia, and one of 10 PHNs in NSW.
Our region stretches from Helensburgh in the north to the Victorian border in the south and inland to Cooma / Monaro, Queanbeyan, Yass and Goulburn.
Our area
We are a large and diverse region, stretching across 11 local government areas and one territory. This includes approximately 680 towns, villages and localities spanning over 50,000km2.
Our region is supported by two Local Health Districts (LHDs) – Illawarra Shoalhaven LHD and Southern NSW LHD.
Our reach and connections help us improve the health, care and wellbeing of one of NSW's largest rural and regional populations.
Our people
South Eastern NSW has a total population of 641,968 people and over half live in major cities, with the rest of the population living in inner and outer regional areas.

Regional profiling and needs assessment
To support our approach to population health planning, we analyse and interpret a wide range of data, topics and variables.

Sentinel Practices Data Sourcing (SPDS) project
We believe data collected during patient visits to their general practitioner (GP) has great potential for regional population health and service planning.
To explore this, we are studying ways of using electronic health records data for primary care planning. This work is called the Sentinel Practices Data Sourcing (SPDS) project, and has been ongoing since 2013.
